Before scheduling your appointment, please take note of the following:

• Be sure that you have active insurance on file.

• ONLINE SCHEDULING IS NOT AVAILABLE FOR PREGNANCY APPOINTMENTS. PLEASE CALL OUR OFFICE TO SCHEDULE A PREGANNCY APPOINTMET.

• Schedule your annual exam at least one year and one day from your previous annual exam.
